Free CAGE Questionnaire for Substance Abuse Therapy
The CAGE substance abuse screening tool* is a questionnaire used by clinicians including psychotherapists, psychologists, and psychiatrists to identify alcohol use disorder.
This is a four-question test that is used by mental health professionals to identify potential substance abuse issues. On page 2, you’ll find the CAGE-AID Questionnaire, which has been slightly reworded to include drug use in the screening in addition to alcohol use.
